How to register

Patients living within our practice boundary who wish to register with the practice will be given a 'new patient pack'. This pack will contain information about the practice and explain fully the registration process. Wherever possible, new patients should bring their medical card to reception.

You may download a copy of our standard registration form and email it to worthingmedicalgroup@nhs.net. After you have emailed the completed form to us, you should arrange to come into the surgery to collect our information sheets and leaflets. We will respond to you email as soon as we can, confirming whether you have supplied enough information to be added to our register, and if you need to have an initial appointment with one of our clinicians.
